If you are interested in signing up for a program- Please state which program you are interested in, and answer the following questions:

1) What is your main complaint? 2) How long has this been going on? 3) What have you tried that hasn't helped? 4) This is an opt-in model of self-care that will require money, time, and effort to see change, I write the program but you have to be the one that shows up to see change! I might radically change your diet/sleep/exercise habits. This includes: no processed sugar, no alcohol, no dairy, no gluten, and no caffeine for a minimum of 90 days. I am going to do a serious inventory of what is in your home and office to search for stressors in your life. Tell me your thoughts about this, and do you have the time and space for this?
